ICMS provides intensive, personalized, collaborative and flexible outreach services designed to engage, support and integrate individuals with serious and persistent mental illness into the community of their choice and facilitate their use of available resources and supports in order to maximize their independence. Such services are designed to assist individuals in their recovery by helping them gain access to needed mental health, medical, social, educational, vocational, housing, and other services and resources.

Services Include:
Engagement, assessment, service planning and linkage, system advocacy, and assistance with care coordination

Eligibility	Resident of Burlington, Camden, or Mercer counties.
