Do you know a youth or young adult experiencing significant changes in mood, behaviors, thoughts and/or perceptions? These changes may be symptoms of early onset psychosis. CHRP provides early intervention support services for youth and young adults (ages 10–25) to improve cognitive, behavioral, and social functioning.
As an early intervention model, CHRP utilizes a continuum of care to provide outreach, psychoeducation, and intervention services to prevent further symptoms. Research has shown that an early intervention approach for those at a higher risk for psychosis improves symptoms, decreases overall disability, and improves age-appropriate behavior at home, school, and work.
Services Include:
- Individual, family and group therapy
- Individualized treatment plans
- Psychoeducation
- Medication management
- Case management and assessment
Eligibility | Individuals ages 15–25, that resides in Atlantic, Burlington, Camden, Cape May, Cumberland, Gloucester, or Salem County. |
